· Regular Discharges – MD should be aware per POC, if not, call the MD and document your call to notify DC. DC summary is required for all disciplines in addition to a regular note in which you need to write “MD is aware of DC per POC.”
· Transfer (patient went to hospital) If you complete a transfer OASIS, please bill in Alpine Kinnser to ensure proper pay and billing. The only time you should Discharge following a transfer to the hospital, is if the patient is still in the hospital at the end of the 60 day certification period.
