-Clinicians need to choose only 4 pathways in PointCare: this is under actions>> pathways:

  1. Choose additional visits need to be provided pathway (there is an option to chose eval only if appropriate)>> save

  2. Choose focus of care>>fill in the blanks

  3. Choose appropriate (low/moderate/high) risk for hospitalization and change narrative as appropriate

  4. SOC statement pathway

  5. Click save when complete the above 4 pathways. Validation failure may show up. It is ok to continue as long as the 4 pathways were chosen and completed

  6. They basically want 90% of documentation be completed at home

  7. After leaving patient’s home, call agency to report SOC and POC.

  8. If don’t get VO within reasonable time>>fill agency form and request to fax.
