Insurance specific guidelines: Private pay, Cigna, workers comp, no SOC OASIS - Cigna patients may have a co-pay to meet the deductible, if so this will be added to the insurance line in SOLI in case the patient has questions. Interim will always reach out to the patient to discuss the copay, but the patient may have questions when you see them. Please make sure the patient is ok with the copay when you call to schedule the evaluation, just in case the copay deters them from services.

8. Agency Disclosure Notice Consent

9. Written Notice of Home Care Consumer Rights Consent
