Eval Checklist

  • Determine if appointment needs to be coordinated with nurse for scheduling (PHHC) if yes, give agency available times  and have them schedule patient. If no, proceed to next step.  

  •  Make appointment with patient

    • make 2 attempts to reach patient before letting Team and agency know you’ve attempted contact

  • Alert agency of appointment time/ set up EVV if applicable 

  •  Go to Appointment! 

    • Begin EVV at start of appointment or obtain additional signature  if applicable per agency protocol

    • Get signature in Soli 

    • Eval patient (obtain all vitals)

    • Schedule next appointment depending on desired frequency 

  •  Input frequency in Soli

  •  Obtain VO per protocol, most likely call dr. Listed in Soli request VO for desired frequency and delay of order if applicable 

  • If applicable, inform agency of scheduling and frequency

  • Document eval, include VO with person’s name who gave them, date and time. 

SOC Checklist

  • Make appointment with patient 

  • Alert agency of appointment time/set up EVV if applicable 

  • Go to Alpine protocols and download or print Agency’s admit paperwork. Some agencies have electronic admit paperwork. If this is the case, once you confirm appointment time with them they will email you admit paperwork. 

  • Go to Appointment! 

    • Begin EVV at start of appointment or obtain additional signature  if applicable per agency protocol

    • Get signature in Soli 

    • Sign admit paperwork with patient

    • Get picture of insurance card

    • Complete BIMS and depression screening

    • Review medical history including height and weight

    • Obtain PLOF 

    • Ask about how much help they need for ADLs

    • Eval patient (obtain all vitals)

    • Schedule next appointment depending on desired frequency 

  • Input frequency in Soli

  • Obtain VO per protocol, most likely call dr. Listed in Soli request VO for desired frequency and delay of order if applicable 

  •  If applicable, inform agency of scheduling and frequency

  • In kinnser, go to My patients, select patient, select misc tab, add admission under your name

  • Take pictures of admit paperwork to upload to kinnser if paper copies used. Otherwise upload digital copies. If agency used docusign skip this step. 

  • Document oasis (schedule time with team to go over oasis before submitting) 

    • Cross reference referral and PMH to fill in OASIS

    • Use template of “additional orders”
